Based on what I can observe from your photos, your concerns about the lower third of the face seem related to both volume loss and structural proportions. In such cases, I typically recommend a combination of targeted fat grafting to restore volume and improve contour, along with a lip lift procedure to address the length of the philtrum and enhance lip fullness. These treatments can help create a more harmonious and balanced facial appearance without the need for more invasive surgery. However, a detailed in-person evaluation is necessary to assess skin quality and determine the best individualized approach.

You may benefit from a rhinoplasty to change the tip that is somewhat recessed. You may also benefit from a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.

It is very difficult to give you good advise based on these photos. Rhinoplasty is a possiblity, although you do not voice that as a problem; lip lift may be considered in light of long phyltrum and thin upper lip. Only on the basis of in person consultation, an experienced board certified plastic surgeon could give you a more detailed plan of treatment. Good luck.
