Sorry that this has happened to you. Permanent scarring is a known potential consequences of surgical procedures. From the photos provided, it appears that there are some elevated portions. These can be flattened with intralesional anti-inflammatory injections. For the general texture irregularities and any color irregularities, professionally performed manual microneedling (no need for the added expense and risks of RF microneedling) can be performed. Fractional lasers such as Fraxel, erbium, and CO2 laser can be combined with radio frequency and also fractional RF to improve textual issues and microtopography. Subcision and TCA cross are also used for deep ice pick scars and narrow valley scars in order to improve the micro texture. Above all, our combination approach with our clinical nurses and board-certified plastic surgeons can improve the quality of your skin by using at home Melarase creams combined with PRP treatments in our office to improve your skin quality and tone. Finally, hyperpigmentation of scars can be improved with at-home Melarase AM, Melarase PM, and Melapads to help reduce active PIH and melanin deposition.

Intralesional injections of steroid and 5FU will melt down the scar and zapping with the electric needle to get rid of the little broken capillaries will make a huge improvement.

You will need a scar revision. This can be done in the office under local anesthesia.

Since you are one year after face lift surgery your scars are considered mature and very likely will not improve much. Scar treatment by means of corticosteroid injection and scar revision should be discussed with your surgeon. Good luck.

Kenalog injections to help soften the thick areas and micro-needling could be done overall for improvement.
