I’m looking into getting an implant exchange. I have sientra cohesive 435CC mod+, I love the size but want more projection. What size should I go to in a hp implant? I am 6 months post op lift with implant.
Answer: Change of implant for more projection You should change to a HP implant of similar width which would be in the neighborhood of 550cc. You don't want to go any wider as this will give you more girth without necessarily increasing projection at all.

Answer: Try on sizers to see which one looks best Hello and thank you for your question. From your photos, switching to a high profile implant looks like a good option for you. Though I cannot say for certain without an in-person evaluation, I believe it will deliver the projection you want without sacrificing the elements of your augmentation that you love. As for size, an implant with slightly higher CCs will likely make the most sense — this will ensure you maintain the same width at the base of your breasts while adding volume in the dome of the implant for more projection. You will need to schedule a consultation with your board-certified plastic surgeon to look at the implant sizes available and to compare implant widths. Best of luck!

Answer: Explant and The Bellesoma Lift Exchanging implants is unlikely to give you more projection. The problem is your lift was inadequate and your breasts are not high enough on the chest wall. The technique I recommend is explantation using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation and the ability to breast feed are maintained, if not injured during your previous surgery. At the same time or later, fat transfers can be performed if additional volume is desired. Implants are not lifetime devices and half are removed by 9.5 years. Fat transfers are permanent and incorporated into the body.Best Wishes,Gary Horndeski, M.D.

Answer: Breast implant sizing Great question! Going with the high profile implant will definitely give you more projection. As far as sizing goes, have your surgeon pick an implant that is about the same diameter as the one you have now and looking at the sizing chart you should be able to find the size that best fits your body.Best of luck and hope this information helps!- Dr. Roger Tsai @DrRogerTsaiBoard Certified Plastic Surgeon

Answer: Implant size and type Dear Miamimami2018,Breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small. You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.Daniel Barrett, MD Certified, American Board of Plastic Surgery Member, American Society of Plastic Surgery Member, American Society of Aesthetic Plastic Surgery
