Hello! Since i was a child i broke my nose a few times and never got it fixed. One dr says i have a slight deviated septum. It was head on injuries where the bump is and one dr told me the bones were splayed. Is this true and would this kindof rhinoplasty require the osteotomies?
Answer: What kind of rhinoplasty would this require? Every nose is different and unique to that individual. If the appearance of your nose concerns you enough to do something about it, it can be improved with a natural artistically performed rhinoplasty and probable septoplasty. What would be the best option and exact procedure would depend on an examination and discussion about desired outcome. Excellent results can be achieved with either a closed or open technique. Keep in mind that following advice from a surgeon on this or any other website who proposes to tell you exactly what to do based on limited photos without examining you, physically feeling your tissue, assessing your desired outcome, taking a full medical history and discussing the pros and cons of the operative procedure would not be in your best interest. Natural appearing results need to be individualized and what is appropriate for one patient is not necessarily the best for someone else. I would suggest that you find a surgeon with extensive rhinoplasty experience certified by the American Board of Plastic Surgery and one who is ideally a member of The American Society for Aesthetic Plastic Surgery (ASAPS). If you live outside of the United States, the surgeon should ideally be a member of the International Society of Aesthetic Plastic Surgery (ISAPS) that you trust and are comfortable with. Based on your photographs I believe the cartilage and bone of your nose would need to be addressed, and depending on your difficulty breathing the septum may also need to be addressed. I suspect this would fall within the realm of a standard 'closed' rhinoplasty and you would have an excellent result. However, to discuss the specifics of the operation, you would need to be evaluated and I would recommend you see a board certified plastic surgeon specializing in rhinoplasty. Best of luck to you!

The other is the open approach which involves raising the skin to fully expose the cartilage and bone underneath. When trauma is present, I find it useful to perform open rhinoplasty to fully visualize the damage. Osteotomies are often needed to shift the bones into a more aligned position. Additionally, the septum should be addressed to improve breathing and provide grafting material (cartilage) to keep the nose in a good position. Based on your situation, I would recommend a consultation with a facial plastic surgeon to determine what option may be best in your case.

Answer: Rhinoplasty Your surgery could be done open or closed you definitely need osteotomies done to narrow the bones. Make sure you select your surgeon wisely. The surgeon needs to be highly experienced
