Your photo shows that you have significant lower lid fat prolapse that would benefit from repositioning. You would need a LOT of fat transfer to the tear trough to try to camouflage that much fat and that is a high risk for irregularity or unnatural appearance. You also demonstrate upper eyelid ptosis, which is leading you to subconsciously raise your eyebrows thus creating a hollow upper eyelid. You likely would benefit from upper eyelid ptosis repair in addition to blephaorplasty. An experienced Oculofacial Plastic Surgeon (Oculoplastics) would be a good option for you for a consultation. Laser resurfacing or a chemical peel can be used to help with the fine lines and wrinkles.
negley I appreciate your concern, however in your particular case, you are not a candidate for a fat transfer to the lower eyelid region, but rather conservative removal of fat or repositioning. You have bags of your lower eyelids secondary to bulging fat pockets, a fairly common condition. Consult with a board-certified plastic surgeon in your area to evaluate the best options for you. A lower lid fat transfer is done for patients who have hollow lower lids and no fat. You have a lot of fat in the lower lids, and this can be repositioned into the tear trough with a blepharoplasty with fat repositioning. Then there would be no need for a fat transfer. FAMI is a way of injecting fat into the muscle planes. Fat injection into the lower lids is an advanced technique, with the biggest complication being visible nodules. So only should be done when really necessary.

Fat can be used in many different ways. I don't think there is a difference between patients of different skin/eye colors. It can be used to lighten skin and or decrease shadows/hallows.CO2 laser would not provide the same result. CO2 and fat transfer can be used together.
I suggest you step back and let others do some of the driving. Your best outcome will come from finding the best provider then following his or her recommendations. Look for a senior board-certified plastic surgeon Who specializes in facelift and facial rejuvenation. For example, in my community Issa Eshima and Timothy Martin do some of the best facial surgery. Neither one is cheap but they deliver results consistently that are breathtaking. Facial fat transfer is unpredictable and imprecise. The area around the eyes is by far the most difficult and leaves far too many people posting on real self asking how previously grafted fat can be removed. If surgery is out of the question then I suggest you stay with fillers. Check out the work of Dr. David Marbie when it comes to mid face filler work. I would steer you away from facial fat transfer as the first treatment of choice. I get that people want to take baby steps but the best procedure for you would be a facelift with brow left and upper and lower eyelid blepharoplasty's.
