While I would not charge a fee in this situation, it is within the dentists rights to charge if they want to. I personally think it harms the business, but charging for this is a common thing in some clinics. Many "chain" clinics have low fees, but charge for every little thing that is often included in the fees from another office. If you are at a clinic that is a franchise, or a chain, then I would expect you to be charged for this.

I would not expect a patient to pay for a healing abutment / cover that was ingested . There is a cost to the dentist for a new one but I think it would be unreasonable for the patient to pay . One could argue that it may have not been tightened to a high enough torque allowing it to loosen and so be lost.
