I am sorry you have been going through such a difficult time in regards to # 21. I would recommend opting for a implant if a second attempt at an apicoectomy is necessary. Implants are the standard of care. Please make sure you see a oral surgeon or periodontist to evaluate and place the implant. Good luck.

Hello,

Sinus lifting or augmentation does not increase or decrease the longevity or prognosis of an implant. If the sinus lift is done properly by a surgeon ( either an oral surgeon or periodontist) the implant should hold fine. The bone they place will eventually be converted into your own bone after time. Yes the bone in the maxilla is softer than the mandible however there should be no issues. Go see a surgeon prior to implant placement then your dentist can then restore the implant with a crown. A implant is a better option for any missing tooth.  Best of luck.

The bridge can be supported by a 3.0 X10mm implant, ideally however you would want 2 implants the same size equal distance apart. I would recommend the surgical plan be checked by  prosthodontist ( dental prosthetic specialist ) prior to any further procedure. The restorative plan should be in order prior to any surgical intervention. Good luck.

More importantly make sure the healing cap passed through your intestinal tract. Once you have been cleared  by a Gastroenterologist then proceed with another placement of a healing cap.  Based on the office policies healing caps can be replaced free of charge or have a slight fee, this depends on the office.  Post operative discomfort is absolutely normal, however after 3-5 days post operatively this pain and swelling should be diminishing.  If pain is persisting after this time period see your surgeon again. The white you are seeing is most likely granulation tissue which is part of healing. Make sure you  are rinsing thoroughly with salt water.

I am so sorry you have been experiencing post operative complications. There is a nerve that runs in your  lower jaw. There is always a possibility that this nerve was injured during implant placement.  During pre-operative discussions this is always brought to the paitent's attention. However most of the time the numbness and tingling wears off within 6 month to a year. Please bring it your surgeons attention that you are feeling like this if you do not see improvement soon. I am sorry you are experiencing discomfort. Pain after 2.5 months is not normal. This is usually an indication of  infection. Please see your dental surgeon as soon as possible. Once proper diagnostic images and a clinical evaluation is completed then proper diagnosis, prognosis, and treatment planning can follow.

Implants are almost always a possible treatment option. Age or previous use of dentures is never a contraindication. However because you have dentures for a long time resorption of the bone in your lower jaw is always a possibility. A surgeon would need to take a 3D CBCT x-ray to determine how much bone you have for potential implants.

An apicoectomy is always an option. However an implant may be a better choice due to the fact that it is more predictable in my opinion.

Yes, it is absolutely possible for a second bone grafting procedure to be done at the time of implant placement even if a previous bone graft was done months before. The reason for this being that implants need a certain amount of bone around them in order to succeed. If your surgeon deems it necessary based on the 3-D imaging he or she may place bone at the time of implant placement to give you the best results.
