Based on your photos, I'm still seeing herniated fat under each eye so I'm not sure what was done. You also have a tear trough which can be nicely addressed with a fat transfer. If you prefer no surgery, then a skilled injector could use filler to improve the tear troughs.

It kind of looks like you might still have fat in your eyes... did they do anything to the fat in your first lower bleph? I would check that first. Second, if you can't do filler, then the other option would be to do fat grafting to add volume back.

The message that lower eyelid filler should be avoided because it causes lymphatic blockage is not accurate. What is accurate is that under eye filler service is fussy. Most injectors lack the patience and artistic eye needed to do a great treatment. Done right, under eye filler is fantastic. Finding that fantastic injector is shockingly hard. I have patient who travel to see me for this services from all over the United States and Europe. I can assure you that grafted fat is a disaster for under eye dark circles.
