What type of lower blepharoplasty would be best for my tired looking eyes. Would fat repositioning with transconjunctival approach be best or would I need a external incision to remove skin and tighten then fat transfer

Answer: Brow lift Your eyebrows are too low and it looks like you are frowning. You will need a brow lift to correct this. The incision can be done in the frontal hairline or through the scalp itself, depending on how much excess forehead skin you have. This would help your eyelids and make you look less tired. Answer: Consider A Nonsurgical Robust HA Filler Cheek Augmentation To Improve Under-Eye Bags & Sagging From the photos, there appears to be significant volume deficit in the cheeks. And, since the upper cheeks are a major support for the under-eyes, when there is volume volume deficit in the cheeks, under-eye sagging and bags appear, and there is also deepening of the smile lines below. So... before jumping into expensive, aggressive surgery, fat repositioning, etc. and all the downtime and potential for complications this may engender, you may wish to consider consulting with an experienced, board certified cosmetic dermatologist to explore the use of a robust HA volumizing filler cheek augmentation. Treatiment in experit hands should take about ten or fifteen minutes, with immediate results and with little to no significant downtime.
