Will an underwire bra help stop/correct a breast implant bottoming out? I'm 1 month post op.

Dear Crystal,   Front and side view pictures would have been helpful. Sounds from your description that you have slight bottoming out of your right breast. There are 3 possible reasons for bottoming out: 1. Surgeon over dissected crease.2. The implant is too heavy for the tissue to hold and the implant is weighing down on the crease.3. Superior capsule contracture that is pushing the implants downward., lowering the crease.  For cause 1 and 2  , wearing the wired bra 24/7 for 2-3 months, can help the crease to scar upward, early on, like in your case with slight bottoming out, however, after that period of time,  if the condition did not improve and it is bothering you, inferior capsulorrhaphy (internal bra) will be necessary.   For cause number 3, in addition to the wired bra, a superior implant displacement massage can be helpful too, by reducing the down pressure of the capsule.(all my breasts augmentation patients do the massage anyhow for 1 year)  If, again, no improvement occurs after 2-3 months, superior capsulotomy (scar release) will be needed in addition to the inferior capsulorrhaphy.     Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures in the photo gallery, to make sure that they are numerous, consistent and attractive with nice cleavage, perky, symmetrical and natural looking.                        Best of luck,                                            Dr Widder   

It is unlikely a bra will help protect against skin stretch over the long term. It may help reduce tension now. I think the implant outcome is really forecast in the Operating Room by the surgical procedure, by your anatomic asymmetries (which everyone has) and by your tissue quality which is intrinsic to you and not something you can necessarily manipulate. The bra likely won't hurt so you can certainly try it.

Thanks for your question.  Bottoming out of the breast implant can be a problem.  If the implant descends too far, it will require surgery to move it back to the appropriate position.  If you feel that the implant is descending, it would be best to switch to an underwire bra.  The worst thing that can happen is that the bra doesn't help keep the implant in place, so this is a very low risk proposition.  At best, the underwire will help maintain the correct position of the implant.
