I have lost a lot of volume in my cheeks over the last two or so years, I am 35. I appear dark and/or creased under the eyes and feel my face shape has changed considerably. I am considering facial fillers but will not be able to have significant down time from work so am concerned about bruising. Would I benefit from cheek or tear trough fillers? What is the likelihood of bruising and how long would this last? Many Thanks
Answer: Dermal Filler Hi @ahos122! The upper cheek is a major support for the under eyes. I would first begin with addressing the volume loss in the cheeks followed by addressing the tear trough area. I recommend seeking treatments from expert injectors who have the skill and experience to precisely inject fillers into treatment areas and avoid risks and complications.

Answer: Tear Trough Fillers Based on your picture it appears that you would be a good candidate for dermal filler along the orbital bone to smooth the lid-cheek junction. In this area I like to inject Restylane. You also have some volume loss in your mid face which is nicely replaced with Voluma. The goal is to place enough filler to replace any lose and make a nice improvement while still appearing natural.Good Luck

Answer: Fillers are a good option Thank you for your question and photo. Filler is a great option to treat the volume loss that you are noticing in your malar (cheek) area. The advantages of fillers are low downtime and reversibility. While bruising is possible, using a cannula can decrease this risk significantly. Other options for treating this area more permanently include facial fat transfer. You might consider having some filler placed and see if you like it. If you do, fat transfer would be a more permanent solution. Consider seeing an oculoplastic surgeon in your area for a consultation. Best of luck!

Answer: Cheek or tear trough filler to correct volume loss? Thank you for your questions and pictures. Dermal fillers are a great option for the treatment of facial volume loss as well as for facial reshaping. From your pictures, it does appear that you would benefit from fillers at the cheeks area and the tear trough. Bruising is a common side effect from any type of injection, usually small enough that it can easily be covered with make up. Bruising with dermal fillers, if any, can last from a few days up to a week or two. Using a cannula technique will decrease the chance of bruising and is also safer for the tear trough area. Arnica supplement and/or cream may also help with bruising. I hope this help. Please see an expert for a proper evaluation and treatment plan. All the best, Dr. Michon

Answer: Facial Volume Loss and Tear Troughs Thank you for your question and submitting your photos. You indeed would benefit from volume augmentation in the cheeks, tear troughs and nasolabial folds. Unfortunately, the most common risk is a bruise. This is because a sharp needle that encounters a vessel beneath the skin can cause bleeding. This risk can be reduced but not entirely eliminated with the use of blunt tip cannulas to place the filler. To further minimize your risk avoid taking aspirin, non-steroidal anti-inflammatory drugs like ibuprofen, nutritional supplements, and vitamins. The extent and duration of bruising are difficult to predict, usually 7-10 days to resolve. Best to seek the advice of an experienced injector as selection of filler and placement is dictated by the location being treated and your goals. I hope this was helpful - Dr. Moore.
