I’m a FTM transgender, 42 years old, 5ft 6in high and weigh about 74kg (with my 500cc breast implant). I am looking into getting a BBL done but have a few questions: 1. What’s the best procedure for a male to female transgender to get the contour (hour glass figure) that I wanted? 2. If I’d go for BBL with 360 lipo, can fillers be used if fats are not enough? 3. Where would be the best option to do BBL? I’m from Canada but considering Turkey that always comes up on my search. Thanks
Answer: BBL for MTF transgender Thanks for your question. I have performed many BBLs on MTF transgenders. From your pictures, you seem to be a good candidate for the BBL. The BBL includes 360 lipo, which is lipo of the upper and lower abdomen, the upper and lower back and the flanks. In addition, if you do not have enough fat in these areas, liposuction can be performed in many other areas of the body for fat transfer to the buttocks, such as the upper arms and chin. That said, I do not mix fillers with fat transfer. I recommend you schedule a consult for a proper evaluation. Make sure your surgeon you choose has extensive experience with BBL surgery and clean credentials. Always consult with a board certified plastic surgeon. Answer: LIPOSUCTION AND BBL (DOMINICAN REPUBLIC) Hello dear. For a male-to-female transgender person looking for an hourglass contour, the most common procedures include 360° liposuction combined with a Brazilian butt lift (BBL). These procedures are designed to create a more feminine and curvy figure. Looking at your photos the fat would be enough for the bbl. If you are looking for a surgeon who exceeds your expectations and who has the necessary experience and skills, you have come to the right doctor, I am Manuel Diaz, a certified plastic surgeon, we are in the Dominican Republic. You can write to us at 829-554-6062 to have an evaluation and give you all the necessary recommendations for surgery.

Answer: Feminization with fat distribution manipulation The subcutaneous fat layer is only one layer of tissue. It is not what determines if you have a male or female body. That said both Liposuction and fat transfer can help feminize the body and the degree of improvement is based on your candidacy for the procedure combined with the skill of the provider you select. a BBL is two procedures done at the same time. It is liposuction and fat grafting done as a single procedure. Liposuction not only is required to harvest the fat to use for grafting, but also adds to the results. Patients need to be assessed for the procedure in regards to candidacy. Patients need to be assessed in regards to their candidacy for liposuction and fat transfer. in order to get quality fat transfer results there needs to be a sufficient layer of host tissue to support the grafted fat. They’re also has to be enough fat to work with, and you also need to be in the hands of a provider who has enough skill and experience to deliver quality outcomes and not lose the small amount of precious fat you’ll have to work with. Consistent quality results for these type of procedures is far more difficult than most people realize. The number of plastic surgeons to come close to mastering liposuction and fat transfer is quite small. I cannot emphasize enough, the importance of provider selection. This in my opinion requires multiple in person consultations, and I generally do not recommend people travel long distances for elective surgical procedures, unless absolutely necessary. I do not think you can select the provider properly without meeting them in person. Having only one consultation, more or less eliminates the ability to choose the better provider. I recognize that those who have a limited number of qualified plastic surgeons and community may be challenged to do this, but I still always think that surgical procedures should be done closer to home rather than far away. People do not seem to understand the importance of having multiple in person consultations before selecting a provider. Likewise people seem to not appreciate the number of people need support and issues after surgery, the number of people who will end up needing revision surgery or that complications actually do happen. When a complication happens to you it’s 100% and you don’t want to find yourself thousands of miles away from your provider. I have seen this personally too many times seeing it again. Posting on this website is A poor excuse for postoperative care. Start the process by consulting with local providers in your community. I’m sure with driving distance. You should be able to find some providers who have experience with this procedure. It’s unlikely you should need to get on an airplane to fly for this procedure but in the end people do what they do. The fat you have to work with is all there is. Fillers are not cost-effective and are not going to work as body contouring options. Body contouring Implants have a long list of undesirable side effects, and very high complication rates. I suggest scheduling in person consultations with providers in your community to get an assessment and better understanding of treatment options. I generally recommend people avoid making their own assessments and prescribing their own treatment options without getting the input from plastic surgeons. Best, Mats Hagstrom MD
