I'm 32, 108lbs, 5 feet. I currently have 400cc cohesive gel, HP, round, under the muscle. Mine are 8 years old and they only looked fake for about a year..I want to achieve a big, fake, bolt on look with my next implants. Is saline over filled suggested over silicone? under or over muscle? CC size based on my size? I want to make sure I'm happy with my next surgery. Also do you think removing my natural breast tissue will give it a rounder look too? I had 32C naturally prior to these
Answer: Implants Please do not try and get the round hard balls on your chest with larger implants. They don't look good and do not stay looking like balls but drop down to rocks in socks. Please reconsider because your breasts look fine now. None of the ladies shown have pretty breasts.

Answer: Breast revision Thank you for your question. The large portion of my practice is dedicated to revision breast surgery. To achieve a "fake" look, it would be best to use an ultra high profile silicone implant. Even though it might seem that placing implants on top of the muscle is a good idea, the rate of complications is much higher. In my experience, the on top of the muscle-subglandular placement ultimately results in a slide down the chest within a couple of years giving a very poor aesthetic outcome. Using overfilled saline implants is not a bad idea, however with time, they ultimately result in significantly wrinkling. If you are under the muscle, I would leave them under the muscle and just place a larger ultra high-profile silicone implant. Best wishes to you.

Answer: Implant size Dear kdollxo, breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. You will likely have trouble finding a board certified experienced surgeon willing to help you with this for that reason.
