Endoscopic brow lift or Asian eyelid surgery/blepharoplasty? (Photo)

Question

Can't decide. My eyelids are double and measure around 5-6mm but th skin and fat on top make my eyes look smaller. I also look quite tired and angry which made me speculate maybe a brow lift could be the answer. I get around 25-30 units of Botox in between my brows but it doesn't seem to reduce the angry/tired look. What to do?

Hello lanalang83, thank you for your question and for sharing your photo. In my practice, I perform a lot of Asian blepharoplasty and endoscopic brow lift procedures.  Both surgeries can improve periorbital aesthetics but the indications are different.  From your photo, I think you would benefit more from a blepharoplasty rather than a brow lift.  Your eyebrows appear to be in good position and raising them more might make you appear more "angry" and may not give you the result you are looking for.  Also, botox injections can often times make your glabella appear heavier and may accentuate your angry/tired appearance.  Most people think Botox can lift your eyebrows but injection to the glabella and forehead actually produces the opposite effect. Asian eye repair would improve the appearance of the upper eyelids. The procedure would lengthen the upper eyelid crease and may help answer some of your concerns. Brow position is difficult to assess with only one photo. Prior to considering a brow lift try using Botox over the crows feet area which would help elevate the brows, if you like the improvement then consider proceeding with a brow lift.

appearance. The aesthetic goal of brow-lift surgery should be restoration of youthful brow shape and position, not elevation perse.

It seems that you may be considering opening up the eye without losing your ethnicity.  Based on your description in seems that eyelid rejuvenation is what your considering.  Bring a picture to allow the surgeon to evaluate you as a candidate.  It is very important to discuss what you want but even more important to discuss what you don't want.

Hi and thanks for your question. It appears your eyebrows are in good position so a brow lift doesn't seem to be indicated. You might consider a conservative upper lid blepharoplasty to remove the excess skin and fat from your upper eyelids. A blepharoplasty, Botox brow lifting procedures all have different indications and do not perform the same thing. A brow lift is performed for patients who have a low eyebrow position, which is not the case  in the one limited photograph. An upper blepharoplasty is performed for patients who have excess eyelid skin touching the upper eyelashes. Botox is performed on the corrugator muscles between the eyebrows to prevent the depressor action of the muscle and to help with the scowling look. For many examples, please see the link below

Dear lamalang83, When patients begin to see the signs of aging in the upper third of the face a common complaint is that they feel like they look tired or angry when they are not. If the patients brows are in a good position and they have excess skin and fat in their upper lids then a blepharoplasty is a good procedure. If the brows have begun to drop a little bit then it is best to perform an endoscopic brow lift and place the brows to their pre-aging condition and then decide if there is still laxity in the upper lids. If so then a small amount of skin can be removed from the upper lids at the same time. In your case you would most likely benefit from upper lid blepharoplasty alone as your brow position appears good. Best regards, Michael V. Elam, M.D.
