Hello. I had an upper blepharoplasty with nasal fat pad reduced/removed 6 months ago. However, my right upper eyelid nasal fat pad seems to be protruding even more than before the procedure. Is this residual fat, excess skin, or scarring? I am confused because my surgeon said he does not see the issue. Please let me know how I can correct the bulging in the right upper eyelid. Thank you for your help.
Answer: Slight bulge Hi The slight bulge seen in one of the photos can be a number of things this early in the postoperative stage. One thing it could be is post operative swelling, so I would still give it a full year to heal first. The slight excess can be some fullness from residual skin. The goal is always improvement in plastic surgery, not perfection and it appears there may still be a small amount of skin causing the fold. The other thing it could be is residual fat in the medial pocket. This does not seem likely, as in the photo when you close your eyes the bulge can not be seen on this photograph. It would be best to undergo a full examination to get a better idea, but the likely culprit is a very small amount of extra skin.

Answer: Extra fat following upper blepharoplasty Thank you for sharing your questions and photos. A septum is opened following eyelid surgery. Sometimes addition fat prolapses forward after the surgery is performed. Also it is appropriate for the surgeon to aim for slight under-correction of the problem in order to avoid complications related to over-correction. Your results look good. For additional fat removal see an oculoplastic surgeon who is experienced with the Agnes to remove a bit more fat. Good luck!

Answer: Skin This is hard to definitely say without performing a physical exam. This is some slight residual skin and some residual nasal fat pad in my professional opinion.

Answer: Upper eyelid Thank you for your question. You had a beautiful upper eyelid surgery, it looks almost symmetrical with nice scars. All surgeries have risks of asymmetrical results. After 6 months the swelling should be gone. The bulging tissue can be a bit of skin or fat excess. I would recommend you to remove a small amount of skin and decide to remove more fat during intervention. This is again a pretty good result, you should consider to get a correction when it disturbs you too much.Best luck

Answer: Consider sitting tight for another six months. Based on your photos, your right upper eyelid appears to be more swollen than your left. And since you still have some redness in your scars, this could be an indication that you are not completely healed. If you end up having a persistent asymmetry in the nasal fat area, removing this at the appropriate time is relatively straightforward. I hope this helps, and thank you for your question. Sincerely, Dr. Joseph
