7 years after initial neck lift - what can be done to correct the appearance of turkey neck? (photo)

Question

I had a lower facelift in August 2017, but at the time my surgeon didn't feel I needed a platysmaplasty. Upon review 6 months or so later he did do a 'tweak' (I can't remember what it was called exactly, but it took 30 minutes and left a small 1" scar under my chin). 7 years on and my turkey neck feels worse than ever. What might be the best remedy to improve it? I'm generally happy with everything else, but this I loathe with a passion. Thank you in advance.

Hello and thank you for your question! Your situation is very common in patients that did not have a platysmaplasty with their facelift. The face and the neck should really be lifted together in most patients since the underlying layer that is lifted is contiguous in the face and the neck. It sounds like in your case you still had excess skin laxity and fullness in your neck after surgery, and then a minor procedure was done afterwards which still left you with excess skin laxity in your neck. The issue is that the underlying platysma muscle must be tightened plus there will be excess skin to be removed. The muscle is tightened through the incision under your chin and then the excess skin needs to be removed through hidden incisions behind your ears. It sounds like you may not have had the incisions behind your ears repeated to remove the excess skin, so that is what you are left with. I am sorry you are still dealing with this issue!

Hi from İstanbul, thanks for your question. The sagging you're noticing can be addressed by tightening the platysma through incisions made under the chin and behind the ears. To improve the contour of the slightly hollow areas in the neck, fat injections may also be considered for a more refined appearance. Best of luck to you.

What is observed on your photos is definitely relapse of the platisma bands. I see Aldo a little protrusion of the submandibular glands and may need a little trimming to improve the definition of the jaw line. 
So you are a perfect candidate for Secondary Deep plane neck lift.

Face/neck lifts typically last 5-10 yrs so it is time for another one.  I always do the platysmaplasty with the lower face/neck lift for best results.  Getting it done under local with a little iv sedation for complete comfort is by far the best way to go.
