A rhinoplasty procedure would involve reduction of the dorsal projection and nasal hump. This is accomplished by shaving down both bone and cartilage and narrowing the bridge with medial and lateral osteotomies. Also important make tip adjustments and release the depressor septi ligament, if present, to prevent the tip from drooping upon smiling.
A large nose can berealistically cosmetically reduced and improved with a rhinoplasty.Keep in mind that following advice from asurgeon on this or any other website who proposes to tell you exactly what todo based on limited two dimensional photos without examining you, physicallyfeeling your tissue, assessing your desired outcome, taking a full medicalhistory and discussing the pros and cons of the operative procedure would notbe in your best surgery. Natural appearing results need to be individualizedand what is appropriate for one patient is not necessarily the best for someoneelse. I would suggest that you find a surgeon certified by the American Boardof American Plastic Surgery and one who is ideally a member of The AmericanSociety for Aesthetic Plastic Surgery (ASAPS) or facial plastic surgeon(otolaryngologist) that you trust and are comfortable with. You should discussyour concerns with that surgeon in person.Robert Singer, M.D., FACSLa Jolla, California
