What could be causing swelling, pain and redness 2 years after rhinoplasty?

In your narrative, I do not see the location of your nose where you are seeing redness and swelling. If it is in the nasal tip area, it could be from a permanent suture if that was employed by your surgeon two years ago. Or it could just be an epidermal inclusion cyst. Hello. It's hard to say without photos, but it could be an infection. It may simply be confined to the skin. If there's an issue like this, it's always worth checking with your surgeon. An antibiotic may be all that is needed. Hope this helps.

If you’re having acute onset of pain in your nose, then it’s probably not related to having had surgery two years ago.
Acute onset of tenderness with redness and swelling is highly indicative of a bacterial infection.
I suggest seeking medical care.
