When you have a revision to change the placement from under the muscle to over due to animation deformity should the surgeon use the existing implant or swat the old one for a new one?

Yes, I would highly recommend a revision including downsizing implants (you can see that the current implants are too large and have stretched the scars) with mastopexy and possibly capsular tightening. You may also benefit from an implant support system such as Galaflex.
