I think you for your picture. This may be a complex fix. Certainly a clinical exam would be warranted. please check out a board-certified surgeon who has extensive experience with chin surgery.

The plate and screws can be safely removed and that can generally help with the indent or dimple. I have seen cases like this in the past and removing the hardware can give improvement. It appears that you have a deep mentolabial fold with a large chin pad. You may benefit from an extraoral approach to remove the hardware and simultaneously shave the bone and perform a "chin tuck" of the soft tissue. More photos would be helpful.

You could consider a genioplasty revision. (I would need to see pictures from different angles to give you the best advice) I doubt removing the plate will change the overall appearance of your chin. Since the bone is now completely healed, a revision can be safely completed to get you closer to your desired aesthetic goal. Depending on the bone itself, you may need to have some contouring done as well to get the best result (these are done together).
