Genioplasty, Chin reduction (vertical) without the use of screws/plates, is that possible?

I am unhappy with the vertical length of my chin. I would like to reduce the length by 1-1.5cms. However the idea of having screws and plates and other medical hardware permanently "installed" does not sit right with me. Is this possible via shaving down the bone of the chin alone without the use of hardware?

It is possible to reduce the vertical height of the chin through a submental skin incision and this works in cases that do not require significant horizontal changes. Some patients require vertical reduction and either advancement or setback and this can only be accomplished with an intraoral genioplasty.

No, you don't need plates and screws for the most part. You also may not need as much reduction as you think.  The key is to remove the right amount so that your skin and muscles help you create the look you want.  We do this operation several times a week and rarely need metal.

Reduction genoplasty can often be performed by removing bone from the lower end of the chin. This avoids the need for any plates or screws. A reduction of 1.5 cm is quite a lot, and might be more than necessary. An assessment by a craniofacial surgeon or oral maxillary surgeon would be necessary to determine the best technique for you.

The submental approach is the one technque to vertically shave the chin which does not require plate and screw fixation. This also allows soft tissue management of redundant soft tissue chin pad tissue. Reducing the vertical length of the chin by as much as 15mms may be over estimating what you need or can be done.

There are two aspects to a sagging chin. One soft-tissue eccess the second is a bonus contour excess 
 .Both of these problems can be addressed through a sub mental approach and depending on the anatomy and/or contour deformity it may   be accomplished without any plates or screws.
