Breast lift with fat transfer, how many CC’s can be injected into the breast?

Question

I’ve had my breast implants for over 14 years and I want them removed (they don’t bother me however I just don’t want anymore surgery down the road I’m over it!) I will be getting a tummy tuck to repair my muscles and liposuction. I will remove my implants and do a lift at the same time and fat transfer to my breast. How many CC’s max can be injected? Prior to surgery I was a full B after my implants I’ve been a DD (Victoria secret bras) what is realistically expected after this a C cup?

These procedures can be performed at the same time to improve shape and size discrepancies. In general, 200 cc per breast is typically the upper limit of fat injection recommended by most plastic surgeons.  It is not that more cannot be injected; but on average, this has been determined to be the most that can be expected to live during one transfer.  Multiple transfers would be required for additional cc's.  In general, the abdomen and thighs are preferred donor sites but fat from other areas can be used.
Some of the retention rate will depend upon the amount of fat transferred, as smaller volumes will likely have a higher survival.  Occasionally, a patient can resorb a great deal of the fat despite optimal transfer procedures. I typically transfer from 250 cc to 400 cc or so per breast.  Thinner patients are outstanding candidates in my opinion, because the fat transferred is extremely resistant.  
Results are largely dependent upon the skill of the surgeon, his/her ability to remove fat without creating contour deformities, and your expectations. You would need a consultation with a board certified PS to determine your available fat stores.

Hello and  thank you for  your question and sharing your photos. Generally speaking, you can expect a cup size increase in your breast volume after fat grafting. There is a certain level of uncertainty in how much of the transferred fat will survive, which makes this procedure somewhat unpredictable. The technique I recommend is explantation and lift using The Bellesoma Method.  The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery.  At the same time or later, fat transfers can be performed if additional volume is desired.  If you were a full B before then fat transfers of approximately 1500 cc can make you a C.Best Wishes,Gary Horndeski, M.D.

To most accurately answer your question would require an examination. However, in general, if a significant amount of your breast volume was due to implants and you are removing them, it is unlikely that you will be able to restore that volume with fat grafting alone. Usually , to achieve significant results with grafting, multiple sessions will be required. When performed in conjunction with implant removal and a lift the risk of potential complications such as graft necrosis or infection is higher. I would recommend allowing the breast to heal completely after the lift prior to any grafting.

When it comes to the amount of fat that can be transferred it depends on the size of the breasts and the quality of the tissues. Typically a woman can expect to get 1/2-1 cup size increase in volume with a fat transfer, however in explant cases it can be less because the tissue has been stretched out.
