So I had a BA 10 yrs ago with saline. Since then I had a son, and breast fed. I have totally lost upper pole fullness but also I think they've slid down my body. They don't look as perky as they used to. Do I need a new pocket & bigger implants or just new implants? One dr wants to charge $12k for a revision with sutures for s revised pocket(no strattice) and another doc says I only need bigger implants as he believes sutures eventually give way.

If, on physical examination, your breast implants have “slid" away from ideal position on your chest while, you may benefit from adjustment of the breast implant pockets. This procedure will likely involve internal sutures, capsulorraphy surgery. Unlike what you were told by one of your consultants, this procedure can be a long-term solution for many patients. Based on your communication with your plastic surgeon, the best breast implant size/profile can also be utilized.
You may benefit from additional in-person consultations by board-certified plastic surgeons. Make sure that the surgeon you select has significant demonstrable experience with revisionary breast surgery. You may find the attached link helpful to you as you learn more about options.

It's difficult to know what your concern is, but it appears that the crease beneath your breast has descended with time. This would result in loss of upper pole fullness, and an increase in distance from the nipple to the inframammary crease. If this were the case, you will need replacement of the implants, and reconstruction of the crease in the correct position. You may not need a new pocket to be created. Bigger implants may not be a good idea, because the added weight may place the crease under additional strain, leading to a recurrence of your issues.
