6 Months post lower bleph with fat transfer - puffier than ever. Would a revision even work? (photos)

I had upper and lower bleph done 6 months ago. The under eye bags are worse than ever. At first my doc thought it was inflammation, then residual filler, and now dense transferred fat . We’ve tried hyaluronidase, 5fu, and cortisone shots. At this point I am freaking out as it looks worse than ever. Thoughts? Would a revision even work? I am 47.

You have a problem. Efforts to date are making your lower eyelid post-surgery, post-procedure injuries worse. Your current doctors may mean well but I caution you to not rush into further treatment. I am in the business of trying to fix the most complicated post aesthetic surgery eyelids. Grafted fat in particular is an over promised service that causes very challenging issues due to complex scaring in the lower eyelid and cheek glide planes. Steroid injections do reduce inflammation but it also kills fat. To render an opinion regarding such a complicated situation, a detailed, in person assessment is essential. Anyone who claims they can assess you from a zoom consultation is not being honest with you or themselves. You have other post surgical changes but I full understand that the lower eyelids are your biggest concerns.

You likely need surgery to remove fat lumps but best to have more detailed evaluation with an expert to determine.

Revision surgery is likely your best bet at this point. I would typically remove additional fat and recontour the fat transposition if necessary.
