Breast revision surgery can be very challenging, especially when the use of ADM such as alloderm and strattice are used. Having said that, there are many plastic surgeons in Orange County that would be competent to perform such surgery.There are a few aspects that you need to look at before choosing who the right surgeon is for you. First I would make certain that the surgeon you see is board certified by the American Board of Plastic Surgery. Second, I would look at the reviews online as well as the before and after photos you see on their site. Strattice is a type of graft material used for soft tissue and implant support. It can be very useful in certain circumstances, but presents a number of additional risks, as well. Another option that I have had significant success with in revision augmentation is SERI silk mesh. The silk is gradually replaced by your body's own collagen, leaving a supporting layer when it dissolves. I have found SERI to be significantly easier to work with -- it drapes better within the breast, is more readily incorporated into the tissues, is easier to suture and trim, doesn't necessarily require drain placement, and costs less.My recommendation would be to find an experienced, talented Plastic Surgeon who does a great deal of revision work and see what they would recommend. Don't necessarily look for someone who will use Strattice, look for someone you have confidence in and trust their opinion.
You should go on several consultations with different physicians and see their before and after photos of patients like you. No surgeon knows what another surgeon's practice is completely. That being said, there are authors and speakers who are known in the surgeon community for these procedures. You can ask your surgeon for a referral to such an expert in your area. One such provider in southern California is Steven Teitelbaum, M.D. He presents on such topics as difficult breast revision cases. Though I do not personally know him to recommend him, I've heard colleagues speak highly of his surgical outcomes. See a number of surgeons and pick the one you are most comfortable with! I have used Strattice in breast implant revision for the following cases:a. Implant stabilization to support an implant, when the breast skin's elasticity is not reliable, such as in augmentation-mastopexy;b. Camouflage of lateral pole wrinkling and rippling;c. Asymmetry of the inframammary folds (where the breast meets the chest wall);d. Repair of symmastia (uni-boob);e. Attenuation of the pectoralis muscle lateralization with flexion;f. Camouflage asymmetry when chest walls are noncongruent (uneven), so implants displace differently;g. Mitigation of capsular contracture.Of all the indications, the last is probably the least successful but seems to elongate the time to recurrence. Complications can include:a. Seroma;b. Red breast syndrome;c. recurrence of the original condition. Regrettably the cost of Strattice is quite high, so prior to choosing an OC specialist, consider whether you absolutely need it. Thank you for the question. The use of acellular dermal matrix or biosynthetic mesh may be very helpful when it comes to providing additional support for breast implants and/or breast tissue. I find this material very helpful in cases where patients have recurrent implant displacement concerns and/or significant loss of skin/tissue strength.Acellular dermal matrix is a biologic implant that carries the ability to become integrated into native tissue. It is made by taking a full thickness section of skin from a donor source (his human, porcine, or bovine in origin). These materials may be helpful as a supportive matrix, help with the breast implant displacement/malposition, provide additional support where a capsule repair (capsulorraphy) is carried out ( such as cases involving bottoming out, lateral displacement, symmastia) etc. I have also found the material to be very helpful when it comes to treating severe breast implant rippling/palpability concerns. Furthermore, the acellular dermal matrix is very helpful when it comes to treating challenging/recurrent cases of breast implant encapsulation. You may find the attached link, dedicated to specifically to the use of acellular dermal matrix for breast surgery, helpful to you as you learn more. I would suggest starting with the American Society of Plastic Surgery and/or the Aesthetic Society of Plastic Surgery to obtain a list of well experienced board-certified plastic surgeons.Then, I would suggest you visit a few surgeons whose practices concentrate on aesthetic surgery. Ask to see lots of examples of their work and preferably speak/see patients who have had similar procedures done. Then, once you feel comfortable that your plastic surgeon has extensive experience helping patients achieve the types of outcomes you would be pleased with in a safe fashion, it will be important for you to communicate your goals carefully. Be very careful with your decision-making. You will find, while doing your due diligence, that there are many different “specialties” who will offer their services to you; again, I strongly recommend you concentrate on surgeons certified by the American Board of Plastic Surgery.
