UHP implants. Have they dropped too much? (Photo)

Question

A year ago I had a breast lift with implants high profile 450CC’s under the muscle. My left implant bottomed out so I went in for a second surgery 6 months later to put permanent sutures to fix the bottoming out problem and also went with larger implants (700CC) UHP which bottomed out again not long after I had the surgery. I’ve lost 100+ lbs over the last 5 years and I’m wondering if that’s why my implants are dropping so low? My skin elasticity? I love the way they looked initially.

Thanks for your question! Based upon your photos and my experience of doing thousands of breast augmentation surgeries, I’d say that at this point, you are going to need a revision with reinforcement of the inframammary fold with ADM (acellular dermal matrix, like Alloderm), & you might want to consider downsizing the volume of your implants: I think that their weight & your tissue elasticity is going to continue to be a problem. It does appear the implants have at least on one side bottomed out again. This is not an unusual problem with implants this large or tissue which has lost its elasticity. It is very possible that your tissues have lost a great deal of their elasticity with your significant weight loss. In that scenario larger implants will tend to bottom out more because those larger implants weigh more. Activity level can also play a role. Finally, the type of repair may play a role.  It appears the you had a circumareolar mastopexy as well. You will almost certainly need a revision of the capsule. This could be done with the cautery technique or a combination of cautery and suture or even use of acellular dermal matrix as a support sling.  If there is still an issue of skin laxity then you will need a revision mastopexy. You might consider a vertical mastopexy or an inverted T mastopexy.
