I've had two Facelifts and Platysmaplasty, one year apart. First was a complete failure. 2nd went to a new surgeon and had a complete re-do. Its been 4 months from the 2nd surgery, face and jaw line turned out fabulous, but my neck bands have returned, after the 1st month, the doctor said was too early to tell, but now at 4 months they are still there. How do I approach my doctor about this and is there a fix or am I just destined to always have these aging bands? I do not want Botox.

Answer: Platysmal banding must be corrected surgically The key to correction of platysmal banding is identification preoperatively. These areas can then be marked and a proper corset platysma plasty performed in surgery via a submental or under the chin incision. Over the years, I have become more aggressive in management of the neck and correcting platysmal bands is both highly satisfying for the patient and rewarding for the surgeon. I would seek out a surgeon who has experience in correcting this condition and has photographs to support it. Botox will only temporarily mitigate the problem and laser treatments or Ultherapy have minimal impact and are therefore not recommended in our practice. Best,

Answer: Neck Thank you for your question. I am sorry about your disappointment after surgery. Fist able the SMAS facelift will not treat those bands the platismaplasty will but there are different techniques to correct the bands and I believe you need a transection combined with middle suture of the muscle and to my experience is the most reliable procedure.
