I had lipo gone wrong a couple of years ago and it made my buttocks drop, can someone recommend what’s best for my problem? Should I do a buttock lift or just cut the skin under the butt? Please help

Answer: Revision Liposuction and Fat Injection Hi, This is one of the very difficult problems to be corrected completely. In my opinion, this has to be addressed with revision Liposuction and Fat Injections. You must have realistic expectations, which means accepting the fact that you might not have significant improvement. The other option is to perform posterior thigh lift through the infra gluteal fold, but this will create a scar in that area, so it would be my second choice, only if the first suggested one wouldn't work.

Answer: Lower Buttock Tuck for banana rolls... First, a buttock lift, from the top of the buttock cheeks, will not help at all so this is not an option. On the other hand, lower buttock tuck is kind of like a tummy tuck for the buttock in that directly removes the banana rolls and/or loose sagging skin from not only the lower buttock but also the back of the thigh to tighten both areas and create a more defined buttock crease. In doing so it also aesthetically shortens and/or rounds out the buttock cheek. It is ideal for those patients that have had significant weight loss or fluctuations, changes from fillers injected, multiple pregnancies, banana rolls, banana roll liposuction complications, or who’s derrieres have succumb to the dreaded effects of gravity and aging. It is also effective for those whom feel their buttock cheek is naturally too long or square and just want it proportionately shortened and rounded, respectively. A relatively few number of Board certified plastic surgeon’s perform this procedure on a regular basis so you may have to travel far and away from home to seek consultation with this particular surgeon specialist. Glad to be of help.

Answer: Lipisuction of the banana roll The banana roll should not be treated with liposuction. The bulge or skinfold that patients complain of is related to skin and is not corrected by removing fat. Unfortunately it was a mistake to have this area treated in the first place. Providers with sufficient experience typically learned this throughout their career and in my opinion it should be considered standard of care to avoid doing liposuction in this area. I’m curious if you had any other procedures like fat transfer to your buttocks as well? There are no easy solutions. Generally speaking the two options are to cut out skin under the buttocks or to restore the volume through multiple rounds a fat transfer. If you choose that option then be prepared to need at least three or four rounds a fat transfer and it may not be possible to restore you to pre-procedure. Complex situations like your own always require in person consultations to make a good assessment. Follow up with the plastic surgeon who did your procedure and other plastic surgeons in your area who have extensive experience with both liposuction and fat transfer procedures. Expect different plastic surgeons to have different recommendations. Best, Mats Hagstrom MD
