Hello. Oftentimes the appearance of your knees can be improved by laser liposuction (Smartlipo) of the fat deposits surrounding your knees. Consult an experienced BCPS. 

What is it about your knees and legs that are bothering you? If it is a thickness issue, this can easily be treated with liposuction to the thighs and knees for a more contoured look. I cannot tell from you picture if your skin is loose. If so that would be an inner thigh lift, which would tighten as well as lift the skin in the upper leg/knee area. There is no need to cover the area with tattoos.
