Thank you for your question. It can be a difficult problem to correct and multiple maneuvers need to be undertaken. Your implants need to be downsized at least till 310, but possibly even smaller. Galaflex will help prevent the implant from moving within their pocket. Additionally He will need to make the pocket where the implant sits smaller. All these maneuvers should help to fix the symmastia as well as to media lies your nipples which Appear even more lateral due to the enhancement of the brass which is like magnifying any discrepancy preoperatively. I recommend removing the implants from the pre-pectoral position and placing them submuscular as well as a smaller size. You would also benefit from a mini lift. Using a circumarelola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage.

breast revision surgery is the most difficult plastic surgery procedure performed. That sounds like a good solution. You will need to avoid wearing bras that push your breast together. You need bras that separate your breasts.

While this can be much improved, my approach would be different

Your implants are sub glandular. Are you sure.? This is unusual over the muscle.

The repair may work in one try.

I would stop with a poor result, remove the implants and capsule, suture tissues back into normal position and allow healing.

Then if your anatomy proves correct replace the implants under the muscle in a new pocket.
