Your breasts are much too low on the chest wall and the lift was inadequate. At this time, I recommend explantation with lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, smaller implants can be placed totally submuscular.

Your implants are too large for your frame. You need to go smaller, like 400 cc's, so the pocket does not stretch from the weight of the heavy implant.
