Hello and thank you for your question. Based on your photographs, you may benefit from a revision in order to de-rotate and refine your nasal tip. This can be accomplished by releasing the scar tissue and then placing extended spreader grafts to de-rotate the entire tip with less nostril exposure. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Revision rhinoplasty is more difficult than a primary rhinoplasty, so a full set of facial photographs from all angles are going to be required to make a decision about how best to proceed. A copy of the operative report would also also be helpful to understand what was accomplished in the first procedure and it's very important to know how much cartilage is left over for grafting purposes. You have an inverted upper lateral cartilage and a wide nasal bone on your left side which is evident only from the front view. An Osteomy placed the nasal bone and a spreader graft will be required to improve those issues
