Hi, i have had two augmentations, an initial and corrective surgery. Today after taking iff my bra i joticed ine imomant seem to be flipping. I have dual plane, but it’s almost like one came out of hnder the muscle and now is flipping sideways. what can i do?

Implant malrotation is a function of a mismatch between the implant volume/size and the pocket volume/size. This can be a function of overly aggressive dissection in the short term or tissue stretch in the long term.

This can be addressed definitively via implant exchange and upsize or capsulorrhaphy to shrink the pocket. In the interim, the implant can be manipulated and repositioned externally.

Thank you for sharing your photos and concern. What you are describing &mdash; the sense that one implant has shifted out of its pocket and is turning sideways &mdash; most often points to either implant displacement or a disruption of the dual-plane pocket, sometimes with partial loss of muscle coverage or a capsular issue. Round smooth implants don't have a "wrong" orientation the way a teardrop implant does, but they can certainly migrate within an over-stretched or torn pocket, which can make one breast look and feel different from the other.
Because you've already had a primary and a corrective surgery, the pocket and surrounding capsule have been operated on before, and revision findings are quite individual. The most important next step is an in-person examination, ideally with an ultrasound and, if needed, an MRI &mdash; both to assess the integrity of the implant itself and to map the pocket and any capsular laxity or muscle release.
If a true malposition or pocket problem is confirmed, the corrective options typically include a capsulorrhaphy (internal stitching to reshape and tighten the pocket), often reinforced with an acellular dermal matrix or mesh for added support, repositioning to a fresh plane, or implant exchange.
