Hello I am 5 2 and 145 pounds femail. Aftwr 3 pregnancies i kbow i need a lift but have a hard time choodinf implamt size. My dr says he will be uaomg rhe sizer once im on the operating table and at consultation he literaly just said i wouls be gopd with 275cc ...i want to remain natural but i dont want to regret if breasta turn our to small eithee. Bra size is 38D

Implant size cannot be chosen accurately from height, weight, bra size, or one photo alone. The right range depends on your breast base width, tissue thickness, skin looseness, how much lift is being done, and the final shape you want. Since you already know you need a lift, going too large can sometimes make the result look less natural, place more weight on the lift, and increase the chance of recurrent drooping over time.

A 275 cc implant can be a reasonable natural-looking range for many patients, but it is not a promise that it will be right for you. Intraoperative sizers are helpful, but you should still have a clear preoperative plan: ask your surgeon what implant range he expects to use, what cup-size change is realistic after the lift, and whether your goal photos match a small, moderate, or fuller implant. If you want natural but do not want to feel too small, I would focus on choosing the range that fits your chest width and lift plan rather than chasing a specific cup size.

Thank you for your question &mdash; and you're already on the right track recognizing that after three pregnancies you need a lift. I can't be fully definitive from a single photo, but there does appear to be some degree of sagging (ptosis), and I want to be honest about one important point: a very small implant on its own will not take up or tighten the excess, loose skin that comes with this sagging, so choosing too small an implant would not be appropriate and would likely leave you unsatisfied. In fact, it looks like you genuinely may need a mastopexy (lift) as part of your plan &mdash; but to say this with certainty and to choose the right approach, a hands-on in-person examination is essential.
On size itself: the right choice isn't a single cc number &mdash; it's driven by your chest width (base diameter) and your existing tissue. An implant wider than your natural breast footprint tends to look unnatural and can cause problems over time, so a "natural" result usually means matching the implant diameter to your anatomy and then selecting a profile (projection) for fullness. The 275cc your surgeon mentioned is a reasonable starting estimate rather than a fixed commitment, and using sizers in the operating room is actually good practice, because the same implant looks different once the breast is lifted. Importantly, with a proper lift you often need less volume than you'd expect to look full &mdash; so the lift, not just the implant size, is what protects your "natural" goal.
The best way to avoid regret is a dedicated sizing session &mdash; trying on sizers in a bra, reviewing photos of patients with similar height/weight who had a lift, and ideally 3D imaging &mdash; combined with a physical exam to measure your skin laxity and confirm the lift plan.
