I have a super thick muscular looking neck so much so people ask if I work out. I do not. Is there any procedure to feminize my neck shoulder area? I also have very narrow hips which also make me look more masculine. I am a cisgendered woman by the way.

You cannot evaluate the sagging under the chin alone. Because this is usually the last step of sagging in other parts of the face. In addition, chin stretching may cause sagging under the chin itself. Please, if you share full photos of your face here or in private, a full diagnosis can be made.

What cannot be appreciated from the photograph is how loose the skin is on stretch. If it is springy and recoils easily from being stretched then liposuction of the neck will work easiest. If not then a mini neck lift will do the trick.
