It is a little hard to tell with the one photo from above, but it appears your nipple is above you breast fold. You look more deflated than sagging. Probably a dual plane augmentation is all you need to refill your breast and help center the nipple. Seek more in person consults. Good luck.

After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They then get to choose what looks right for them. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Women tell me that this process is very helpful in determining what size and style breast implant is right for them. Mildly Sagging Breasts: If your breasts are mildly droopy, a saline or silicone gel implant placed behind the pectoral muscle might help create the illusion of perkiness. That’s because as the implant fills out the top of the breast, it also fills out the bottom, making it look as if the nipple has moved higher. It hasn’t. Thank you for sharing your question and photograph. Nothing replaces an in-person examination but it appears you could have an implant alone and do well.

The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers or small implants can be placed totally submuscular.

You are an excellent candidate for a submuscular augmentation with saline or silicone implants, depending on your preference. We would determine the exact size and profile when I measure your chest, we review photos together, and you have an opportunity to try on the implant sizers here in the office! I would also recommend a donut mastopexy because your nipples are a little low and widely separated on your breasts. A donut mastopexy would give you a better result!

From the photos I would say you are borderline as far as needing a lift. I would let you make the decision which way you wish to go.

Good morning. I understand the appeal of only wanting an augmentation without a lift. However, without examining you in person, it is impossible to determine your skin laxity and precise breast measurements that would tell if you require a mastopexy along with your implants. If a lift is not performed when indicated, you run the risk of having bottom heavy breasts with improper nipple placement. I would recommend consulting with a few board certified plastic surgeons to be absolutely certain you're receiving the proper surgery for your particular anatomy.
