I’m 6 months post-op a revision septorhinoplasty and I’m really struggling with the rib cartilage septal extension graft that my surgeon placed. In my primary surgery a septal cartilage columellar strut graft was placed in tip, and I never had any functional issues with it. By comparison my current SEG bothers me daily. I can feel it in my nose constantly, even more so when I purse my lips, smile or talk animatedly. I can’t wipe, pick, or blow my nose properly, and when I try the graft clicks awfully. The graft also causes troubles during intimacy, as I can’t kiss my partner passionately without discomfort. Finally, I feel like the graft makes my nose look pointy from the front. If I had known I was getting this graft I wouldn’t have gotten a revision. I only know now because I asked my surgeon directly what they put in my nose. As I understand it, they placed the graft to help slightly derotate and stabilise my tip. I also think they used the graft to augment my infratip, which is not something I asked for or knew they were going to do. I have attached a photo of my birth nose, primary and current result. If I continue to experience these issues, could I have the SEG removed and replaced with a columellar strut graft for support + some other derotation measure e.g extenders spreader grafts or a derotation graft?
