Too much was taken away in a nipple reduction. I made a nipple reduction some months ago and it didn't turn out the way I wanted it to be because now I can almost not see them. I asked the doctor to reduce the puffy nippels but she told me nipple reduction would help... so now I still have puffy nippels, without any nippels.

Usually, in a case like yours, treating the gynecomastia reduces the nipple and areolar size without visible scars. Thereafter, if further nipple reduction is desired, a more conservative nipple reduction can be performed. Once a complete surgical nipple reduction is performed in men, it is a permanent result. Injectable fillers or nipple reconstruction will not restore the natural appearance of a male nipple. These techniques can be used to restore nipple size and contour in women because they are larger. My only recommendation would be to consider tatooing of a faux nipple. This will give you the appearance of a normal nipple without the projection that can show through shirts. It appears that you had an areolar reduction performed but the best procedure would have been to remove any glandular tissue to avoid the extra scarring and minimize the puffiness that can develop. Unfortunately during nipple reduction tissue is permanently removed to shorten the nipple( this can be done as part of gynaecomastia procedures or separately as the length of the nipple is a separate issue from puffy areola caused by gland excess). In your case would be advisable further gynaecomastia surgery to flatten the areola so that short nipple would be more in proportion. If this not enough some nipple reconstruction techniques using local flaps similar to the ones used after breast cancer treatment can be considered.
