Yes, at this point you are seeing the effects of scar tissue. Unfortunately, most of the swelling is gone by 10 months post-op, so your indentations and irregularities are permanent. A secondary procedure involving liposuction and fat grafting should be able to produce more favorable results. The creases you have are due to scarring of the underlying tissue.  When just the breast tissue is removed the skin will scar down to the vacant site. In virtually all cases of gynecomastia I feel that it's necessary to do liposuction in addition to the excision of tissue.  This allows for the skin to redrape so that creases are minimized.  Because your skin already has significant memory in the creases it will likely require a combination of procedures to get it right.
