The appearance of implants can be very different when placed above the muscle vs. under the muscle. There may be advantages to both, but generally symmetry is best when both implants are placed in the same location. To determine the best course of action at this point would require an in person physical examination. The best recommendation cannot be made on photos alone. I would suggest beginning by returning to your original surgeon and discuss your options.

I have seen patients who have implants placed subglandularly experience pre-mature sagging. There simply isn't enough support to prevent the skin from stretching and gravity pulling the implants downwards (especially with a larger, heavier implant).

It is difficult to truly determine exactly what is happening based on one view and without a physical exam. It appears that you likely needed a smaller implant with a lift. I recommend finding a board-eligible or board-certified plastic surgeon with aesthetic fellowship training.

Placing the implants in front of the muscle has lost some support for their weight. One side has bottomed out from the weight of the implant and not much support. Ideally, you should go down in size and probably use mesh plus tighten the inferior pocket. You will not have the upper breast fullness placing implants in front of the muscle like you had with implants behind the muscle.
