I had a mommy makeover over two years ago. At my one-year follow-up, I expressed concerns about the appearance of my stomach, and I was told that the unevenness was due to swelling. However, after all this time, I find it hard to believe it could still be swollen—so why does it still look so uneven?
Answer: Possibly due to liposuction. Thank you for your photos. It appears that the lower portion of your abdomen, below your belly button, is slightly fuller than the upper portion of your abdomen, especially the central portion. I don’t have the benefit of knowing what your original surgeon did, however, it’s possible that liposuction was done during your tummy tuck, and that slightly more volume was removed from your upper abdomen. You could check with your original surgeon about this and see if improved contour could be achieved by a revision liposuction. Best wishes.

Of course it depends on the reason, and you would need a good workup to determine this. If your plastic surgeon cannot help you, perhaps you could get another opinion.

Answer: May need more lipo or a horizontal muscle repair Most people, when they do tummy tucks, do not do any liposuction to the central area of the abdomen as this area is lifted up (what we call undermined) and the goal is to preserve blood flow. So it is possible that there is excess fat there that could be treated now that you have done the tuck to make the skin tighter. The other possibility is that there is vertical laxity of the abdominal wall. Typically the muscle is repaired down the center in a vertical line to bring the muscles back in line. However, there are times where a multi-vector repair is needed to correct bulging. Most surgeons don't even look of this, but it can be a frequent cause of building of the lower abdomen. I do a lot of revision tummy tucks and often this is a big part of what needs to be corrected.
