Best option would be liposuction and fat transfer. The biggest problem will be finding someone who has mastered this procedure.Doing liposuction and fat transfer well is more difficult than most people realize. To find the right provider may require having a number of in person consultations. Look for plastic surgeons in your community, who have extensive experience with both Liposuction and fat transfer.During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients with similar body characteristics to your own.An experienced provider should have no difficulty showing you to before and after pictures of at least 50 previous patients.Being shown a handful of preselected images, representing only the best results of a providers career may be insufficient to get a clear understanding of what average results look like in the hands of each provider, what your results are likely to look like or how many of these operations they’ve actually done. For fat transfer outcomes recognized that early results can look very impressive, but do not in anyway represent final outcomes.Fat transfer results should never be considered final until at least three months from the date of surgery.Never assume that after pictures represent long-term outcomes. Always confirm when after pictures were taken in relationship to the date of surgery. There’s no correct number of consultations needed to find the right provider. The more consultations, you have the more likely you are to find the best provider for your needs.Considering how much variation there is in both Liposuction and fat transfer skills, I recommend patients consider having five consultations or more before considering permanent irreversible body contouring procedures. Being board-certified in plastic surgery with years of experience does not mean somebody is mastered every procedure, especially not difficult operations with long learning curves, like body contouring with liposuction and fat transfer.Majority of plastic surgeons in the United States do not offer gluteal implant augmentation. There are good reasons why most plastic surgeons don’t do the procedure.Gluteal implant is associated with very high complication rates, high failure rates, high revision rates and low, long-term patient satisfaction.Gluteal implants are not like breast implants in anyway. It’s simply isn’t a good operation. For those who do choose gluteal implants it is very important that you select out one of the few plastic surgeons who do this procedure exclusively or have decades of experience, trying to master this procedure. When in doubt slow down and schedule more consultations.Best,Mats Hagstrom, MD
