Hello Doctors, I am 5.7' , 130 lb. Few years ago I had full body lipo and BBL. I lost weigh and now my butt is saggy(lower portion of the buttock). Which procedures will be best for me to achieve lifted buttock ? Implants(oval or round, size), lower butt lift or BBL ?
Answer: Saggy butt after BBL… Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made. Buttock “sagging” is usually a combination of loss of volume or deflation (after weight loss, liposuction, or post-BBL) and/or the effect of aging and gravity. So, I personally first treat patients like you by restoring volume to the buttock with a buttock implant. This will create a more desirable smooth and rounded perky buttock profile. If not adding volume with an implant, the buttock will be shortened but may appear more flat with just a buttock lift. A lower buttock tuck is usually then performed at six months following implants. In this sequence, the implants have fully settled into final position so that any remaining loose skin can be most accurately identified and removed with the lower buttock tuck. Please note, because an extremely few number of board certified plastic surgeons specialize in buttock implants, you may have to travel away from your local area to seek consultation with this particular type of surgeon. Glad to help.

Answer: Reasons to avoid include your implants. Based on the single picture, you’ve included, your bottom is already proportionately large to your midsection. I did not recommend augmenting at your bottom further, if you want to maintain balance and proportion. I’m assuming you had previous significant weight loss based on your history. Sometimes Body contours have inherent, limitations and gluteal augmentation with skin laxity is always going to be a challenge. The majority of plastic surgeons in the United States do not offer gluteal implants. There’s a good reason for this. The procedure has a high complication and very high patient satisfaction rate. Revisions and removal of gluteal implant rates are fairly high. This in conjunction with your bottom, already being slightly on the large side, makes me think that further gluteal augmentation is probably not in your favor. I suggest scheduling in person consultations for more accurate assessment and understanding of treatment options. Best, Mats Hagstrom, MD
