Rhinoplasty under anesthesia - what is safest method if you do a total rhinoplasty, where a lot needs to be fixed?

I asked for a while to see if i can do rhinoplasty under anesthesia with LMA laryngeal mask airway and got the answer that ETT is safer. But my clinic actually always does it under local anesthesia where you are awake and have light sedation. But if I want to do under anesthesia, it will be with LMA. what is safest in that case to choose if you do a total rhinoplasty, where a lot needs to be fixed?. I think mostly safety regarding aspiration, blood running down the throat from the nose.

The purpose of this insistence is to protect somebody's Airway. Since the nose is part of the airway, and since we are working on the inside of somebody's nose all of the time, a bit of blood naturally drops into the throat and this can go into the lungs if there is not an endotracheal to with a cuff on it. General anesthesia is the safest anesthetic method for performing rhinoplasty, and for avoiding a complication like aspiration pneumonia. LMA Is not sufficient. Thank you for this question! In my experience, the safest way to perform a rhinoplasty, whether it is a full rhinoplasty, a tip rhinoplasty or some other nasal surgery, is to do so with a board-certified anesthesiologist utilizing an endotracheal tube (ETT) to reduce the risk of aspiration.

The biggest risk is bleeding and aspiration. If a lot needs to be done the risk of bleeding is higher. The safest airway is ETT. Some anesthesiologists and surgeons feel comfortable with LMA as well. A fewer might do under local. It depends on the comfort level of the surgeon. However, if you had to have one answer, in this case ETT

Based on your text, I would recommend general endotracheal anesthesia over the other options especially local anesthesia is difficult to perform a full rhinoplasty. Best to consult to do a virtual consultation for better understanding.
