My nose is bulbous but too short (piggy) and lacks projection. Therefore I'm interested in both making the tip thinner but also longer. Is it possible to achieve both? (Also augmenting the bridge.) Thank You!
Answer: Augmentation rhinoplasty Thank you for your question. The type of changes that you are describing are compatible and generally involve bringing some cartilage from outside the nose to achieve it. In my practice, rib cartilage is the material of choice for increased projection of the bridge and tip since it is less likely that the septal cartilage or ear cartilage would be reliable to this effect. Inevitably, using rib cartilage would make the tip of the nose feels firmer than when it is now. The firmer scaffolding was increased projection allows the soft tissues of the tip and middle vault to stretch a little bit more and produce stronger shadows. Because our vision is not in 3D it relies on shadows to understand shapes. The surgery to produce a narrower appearance of the nasal bridge and tip relies on creating more abrupt transition between the surfaces on top in the sides of the nose bridge and tip in the same way as shading with make up on the sides produces the impression of narrower. The degree of narrowing achievable will depend to some extent on the characteristics of the skin and how much it can be thinned when the skin is relatively thick. A consultation in person is generally more valuable to give you an understanding of the extent of changes that is achievable and using computer simulation to help you determine how much increase in projection or projection and length you would like to achieve.
