When considering a second revision, it’s important to note that each subsequent nose surgery becomes more complex due to the loss of structural support, scar tissue, and other factors. The importance of experience, expertise, and artistry cannot be overstated. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in rhinoplasty techniques. I think your tip bulbosity and nostril asymmetry need revision. I am not sure whether you have an implant on the bridge or not, but you have asymmetric nostril probably due to deviated caudal septum.There is still a room for volume decrease till 1 year but it is still bulbous. An alar-plasty accomplishes narrowing wide nostrils. It's important to know whether or not you had this maneuver performed in the last two rhinoplasty surgeries. The procedure involves removing a small wedge of skin at the base of the nostrils and narrowing the base itself. This can be done as a standalone procedure under local anesthesia, or as a component of a full rhinoplasty.
Hello! Thanks for your photos and questions. At 6 months, there's still time for swelling to improve. Asymmetries can work themselves out over time with taping and steroid injections if needed. After 12 months, a discussion can be had regarding addressing any asymmetries. For tip work, re-opening the nose allows for the most precision. A nostril reduction can be done in clinic relatively easily. I would give it some more time before deciding. Hope this helps!
