Can I have tiplasty and alarplasty only without the rhino (goretex bridge implant)? (photos)

Question

I was thinking of having my tiplasty and alarplasty done without goretex implant on my bridge. I want the subtle look that would complement my asian features, nothing too drastic. What's your advice doctors? Is this good or bad?

Based on your photos, I think it will be difficult to only alter your nasal tip without also augmenting your nasal bridge.  Some of the techniques utilized in creating a more refined tip is to alter the tip cartilage and grafts which may raise your nasal tip.  This may make your tip too high for your nasal bridge.  If you are opposed to any synthetic implants, you can consider using an autologous graft such as diced cartilage fascia (DC-F).

The primary problem is that like many Asians, your nose is short (can see into your nostrils). Narrowing the tip and nostrils will make this only worse. You need nasal lengthening in combination with tip plasty - the nostril width itself does not look bad. Nasal lengthening almost always requires rib cartilage because you need long thin strips of cartilage to accomplish this. Although donated rib is an option, for younger patients like you I would strongly prefer using your own rib.

A tip plasty and alar plasty can be done without augmenting your dorsum.
This can give you increased tip definition and reduced alar flaring. The changes can be subtle.

I think your bridge looks fine. You don't necessarily augment the dorsum with alloplast. Tip projection looks fine but a little bulbous. I suggest you tip refinement without alar base surgery. Thanks.

I would suggest consulting with some expert surgeons who use digital imaging to show you the proposed results these procedures would provide and see if those are your desired results and if so then refining the tip and alar base may be enough for you. I would not consider the Goretex implant and if you want the bridge built up consider other grafting materials. You also appear to have a weak chin you may want to consider augmentation to help balance your facial aesthetics.
