You will be in a compression garment after liposuction, so the position that is most comfortable is what you should use to sleep. This may take some attempts to find the most comfortable position but you really don't need a pillow unless it is more comfortable.

Ann I appreciate your concern, however most patients recover and sleep fairly comfortably after liposuction with normal prescribed pain medications. Most people describe the pain after liposuction as similar discomfort or soreness felt after a really hard gym workout. If pillows between the legs add comfort, by all means give it a try! Be certain to consult with a board-certified plastic surgeon in your area to increase your chances for the best possible result. Good luck.
This will vary from surgeon to surgeon. Best guidance will come from the PS who performed your inner thigh and ab lipo. His/her post-operative guidelines will detail the protocol you should follow for optimum results.
You can position yourself however you are comfortable after liposuction. You will be wearing a compression garment though.
