The gold standard for depressed crater type acne, chicken pox, and shingles scars is dermabrasion (not micro-dermabrasion) whereby the skin is sand down with a diamond tip burr to the deeper layers leaving a raw surface for about a week before new smooth skin regenerates. Our treatments use combination therapy to improve the epidermis and dermis for most deep scars. The options available for acne scars depend on the character of your scars, in terms of topography and contour. We see atrophic and hypertrophic acne scars, as well as scars that have hypo and hyperpigmentation. We begin each scar evaluation with an assessment of scar quality and subtype. Our main concern is rebuilding collagen and that is why we use a HIDEF protocol that includes combination therapy. Morpheus 8, scar subcision, fibrous release, fractional laser, CO2 laser, Thulium laser, PDL Vbeam laser, and fractional resurfacing can be used in addition to TCA Cross and chemical peels to further improve skin. Thank you for your question, yours is not an uncommon condition so don't feel as if it is strange. Both dermabrasion and lasers such as the CO2 laser would be beneficial. Plastic surgeons are board certified and well trained in these types of procedures as are dermatologists.
These scars can certainly be improved with lasers. I would recommend seeing a dermatologist with expertise in laser surgery as these might require an advanced understanding on lasers.
These are very deep scars on thick sebaceous skin over cartilage - an area that does not respond as well as the cheek skin. I think a combination of CO2 laser resurfacing with sanding of the surface to smooth down edges -this can be done with a mechanical dermabrader, sterilized sand paper or an erbium laser. It is likely that more than one treatment will be needed for optimal results.
