Asymmetry, crookedness, and size - Will this be a difficult fix? (Photos)

Question

I had a rhinoplasty decades ago as a teenager to narrow the wide tip of my nose. I started noticing that my nostrils were no longer the same width a few years later, and now the tip of my nose seems to be twisting in the direction of the larger nostril too. The tip is also still slightly wider and less defined than I’d like. How complicated might it be to make my nostrils the same size and shape, and pull the tip back to center and slightly refine it? I am honestly terrified of another surgery but the asymmetry, crookedness, and size really bother me.

Hello and thank you for your question. Based on your photographs, you may benefit from a revision rhinoplasty. A surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. This can be designed to correct the caudal asymmetry. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

Your nose looks good. 
It is almost impossible to achieve nostril symmetry. Any attempts can lead to taking too much off one side and then going back to take some more from the other side. As this goes on permanent scarring with poor healing can lead to worst asymmetry than before the surgery was done. 
Also, it is very important to remember that nobody will look at your nostrils from below. From the front view the nostrils don't look as asymmetric as from below. Unless the asymmetry is really obvious and disfiguring, it should be left alone.
